Miyokard infarktüsü ve felç, oral kontraseptif ve östrojen kullanımı ile ilişkili olanlar da dahil olmak üzere, riski yüksek tansiyonu olan hastalarda artar. Ayrıca, (ve progestagenlerin) östrojen kan basıncı yükseltmek ve hipertansiyonu kötüleştirebilir, böylece riski bileşik olabilir. Klinik olarak anlamlı kan basıncı artar östrojen tedavisi sırasında bildirilen, özellikle yüksek dozlarda alan hastalarda ya da oral kontraseptif kombinasyonu yüksek ağırlığının aktivitesi olan ilaçlarla tedavi edilmiştir.
